Thank you for your quick response. I have Internet on my phone that one son gave to me as a gift. I live with my other son. They have an inexpensive service and I can't get service on my laptop. I am also having trouble with my laptop so I haven't been able to use it. I can't print off the forms. I do appreciate your help very much, but, you answered in a way that was unprofessional. You shouldn't question my issues with the Internet like that. I am doing the best that I can with my circumstances and I don't really want to be in this position anyway. If your mother calls the SSA at 1-800-772-1213, then they may be able to send them to her. Additionally, if you have access to a printer, you can print off these forms after your download them from www.ssa.gov
You can get one by calling national toll-free service at 1-800-772-1213 (TTY 1-800-325-0778) or visiting your local Social Security office. An appointment is not required, but if you call ahead and schedule one, it may reduce the time you spend waiting to apply.
