Paralyzed Brother on street

My brother has SSI and Medi-Cal and was ran over by car 4yrs ago he is paralyzed, he was in a nursing facility then moved to a place he could be more independent but still have assistance. The benefits he was getting while living in the nursing facility were steady and I assumed when he tranfered to the assisted living his benefits would transfer over and he would be taken care of....but no my brother is on the street homeless in Downey,Ca and I'm in Texas. He has been in and out of ER and I don't understand what to do or how to get him to social security office to find out where his benefits are going or even if he still has benefits?!
